San Diego is a leader in innovating impactful enhanced service models of care to prevent and end homelessness among older adults. By addressing the unique health and housing needs of vulnerable aging adults, this SHLN workshop will describe the effective integration of health and housing models utilized by the Program for All Inclusive Care for the Elderly (PACE) and affordable/supportive housing programs unique to San Diego.
PACE plus housing creates a robust pathway out of homelessness and promotes healthy aging, reducing avoidable hospital and ER use and preventing premature placement into costly institutions such as nursing homes.
Join this session to learn about the model and best practices of partnering with PACE, the vibrant projects that are implementing this model, tenant outcomes, as well as the success factors in developing a supportive housing partnership.
